Smoking Cessation
Description
The University of Chicago Medicine offers individualized smoking cessation services. The Tobacco Cessation Program designed to help anyone quit smoking within two to three sessions, using an effective combination of FDA-approved medications and behavioral counseling. Services include:
-Nicotine addiction assessment using the highly reliable Fagerstrom Tolerance Questionnaire
-Carbon monoxide monitoring at every visit to record differences in participant's carbon monoxide levels before and after smoking cessation.
-Counseling, provided by a trained tobacco treatment specialist, which includes stress management, problem solving and other strategies to help prevent relapse
-Consultation with physicians who have expertise in prescribing tobacco cessation medications
-Support in abstaining from tobacco use after quitting smoking

Eligibility
-For adults who wish to reduce or stop their use of tobacco and nicotine, including cigarettes, cigars, snuff, hookah, e-cigarettes, and smokeless tobacco.
-For those who are unable to participate in group based smoking cessation services
-Accepted insurance plans include Medicaid, Medicare, UCHP, U-SHIP, and most private PPO insurance plans. Contact your insurance provider or call 773-702-3858 to find out if your insurance is accepted.
-Services may be covered by HMO plans if there are no in network providers. A referral from your primary care provider will be required, and should be faxed to 773-834-6761.

How to Apply
Call the psychiatry intake department at 773-702-3858 to request smoking cessation services. Intake staff will conduct an eligibility screening, verify your insurance benefits, and connect you with the appropriate services. Most people are referred to group services, but individual services are available to those who are unable to participate in group services (e.g., those with scheduling conflicts).
One-on-one services are provided on Fridays by appointment only: the schedule changes frequently. For more information, email ethanis@uchicago.edu.
