Description
The Safe Haven Program of Heartland Health Outreach is a HUD-funded program for individuals who have had a history of homelessness, are at-risk for homelessness and have a persistent and/or chronic mental illness. A wide range of supportive services are provided to residents to maintain their safety and well-being in a stable environment.

Eligibility
- Must be referred from Chicago Central Referral System.
- Must be homeless or at risk of being homeless, with no record of violent, arson, or sex crimes.
- Must have been diagnosed with chronic and/or persistent mental illness.
- Must have documentation of homelessness by previous provider.

Payment
How to Apply
If you are already living in a shelter, ask shelter staff to help you access the Central Referral System. If you are not connected to any program, you can drop by an access point to get connected to a Skilled Assessor who will complete this assessment with you and assist you in obtaining shelter if that is your goal. You can also go to Garfield Center at 10 S. Kedzie or call 311 from a hospital or police station to be connected to a shelter.
