How to Apply
Call 773-205-3637 to request services. A family advocate will speak with you to determine your need for services and schedule accordingly. The crisis nursery has a limited number of openings each day: families with an urgent need for services receive priority. Respite services are available, but they must be scheduled in advance and are generally not provided on weekends.
Bring the following to the Crisis Nursery when you drop off your child:
- child's medical records
- child's immunization records
- insurance information
- any medications in their original container. Medications not in their original container must be accompanied by a doctor's note verifying the medication.
