Eligibility
-CFHC accepts the following insurance plans: Blue Cross Blue Shield, CountyCare, Harmony, Humana, IlliniCare, Medicaid, Meridian, NextLevel Health,United Healthcare, and PPO plans through Aetna, Blue Cross Blue Shield, Cigna, and Humana.
- Accepted dental insurance plans include DentaQuest, Envolve Dental, and Liberty Dental.
-Health care services (excluding dental) are available on a sliding scale basis for patients without insurance.
-This provider participates in the Vaccines For Children (VFC) program. If your child is 1) less than 19 years old and 2) uninsured, Medicaid-eligible, an American Indian or Alaska Native, or underinsured, they are eligible for free vaccines through the VFC program. Click here for more information.

How to Apply
To schedule an appointment, call 773-768-5000, option 2, and request an appointment at the East Side location. Please arrive 15 minutes prior to your appointment time to register and bring the following information:
-Valid ID (e.g.,driver's license, state ID, matricula card)
-Social Security card or alien registration card/”green card”
-Current utility bill
-Current address and phone number
-Medical card or insurance card,if applicable
If you don’t have a medical card or insurance, please bring income information, such as:
-Two paycheck stubs
-W-2 form
-Medicaid card
-Food stamp card
-Unemployment notice
