Call 312-337-1073 and press option 2 to schedule an appointment. Appointments are strongly encouraged. Same day appointments are often available.
If you are a new patient, please arrive at least 30 minutes before your appointment time for registration. If you are a current patient, please arrive at least 15 minutes before your appointment time.
If you are insured, bring proof of insurance and identification (any photo ID or a utility bill with your name and address). If you are uninsured, bring identification and proof of income (two pay stubs, W2, proof of SSI/unemployment/disability benefits, or letter of non-support).
