Near North Health Service Corporation is a 501(c)(3) non-profit Federally Qualified Health Center (FQHC). It is one of the largest providers of community-based primary care in Chicago. The clinic provides health care, social services and nutrition education to the medically indigent and uninsured residents of the Near North side (Cabrini Green), West Town, Humboldt Park, West Garfield Park, Austin, Kenwood/Oakland, Douglas and Grand Boulevard, and Uptown communities.

How to Apply
To make an appointment, call 773-548-1170. Same day appointments are often available.
If a current patient, should arrive 15 – 20 minutes before the appointment time for registration. If a new patient, should arrive at least 30 minutes before the appointment time for registration.
What to Bring to Appointment
If insured, please bring:
1.) Identification
- Identification is required. Clinic accepts one or more of the following types of identification: Driver’s License, State ID, Employment ID, Public Aid ID or Utility Bill indicating name and address of patient, parent or guardian
2.) Proof of Insurance
If uninsured, please bring:
1.) Identification (same as above).
2.) Verification of Income a) If employed, two recent check stubs or W2 Tax Statements, b) If unemployed, Proof of Income (Weekly, Biweekly, or Yearly): SSI benefits, Unemployment benefits, Disability Benefits, Child Support payments with copy of child support agreement, or c) If patient doesn’t receive benefits, should bring a notarized letter of support or non support.
